How NDIS Funding Aligns with Momentum Support Pathways

 

How Our Supports Align with NDIS Funding


Momentum Support Pathways delivers capacity-building supports that translate goals into practical, real-world outcomes.


Our supports align with common NDIS capacity-building categories and are focused on independence, participation and functional progress.


1. Coping Skills & Behaviour Support


Focus: Emotional regulation, behaviour strategies, routines and executive functioning.


NDIS alignment typically includes:


  • Improved Daily Living
     
  • Improved Relationships
     
  • Behaviour Support (when guided by a Behaviour Support Practitioner)
     

Outcome focus:


Participants build practical coping skills, improve behaviour responses and develop consistent routines.


2. Learning Engagement & Participation


Focus: Re-engaging with education, building learning routines and participation.


NDIS alignment typically includes:


  • Improved Daily Living
     
  • Increased Social & Community Participation
     

Outcome focus:


Consistent engagement in learning (school, alternative education or training) and growing independence in participation.


3. Work Readiness & Transition


Focus: Preparing for employment, training and adult life.


NDIS alignment typically includes:


  • Finding & Keeping a Job
     
  • Increased Social & Community Participation
     

Outcome focus:


Clear pathway to employment, training or structured daytime activity with sustained participation.


4. Psychosocial Recovery & Wellbeing


Focus: Mental health recovery, stability, confidence and community connection.


NDIS alignment typically includes:


  • Improved Daily Living
     
  • Increased Social & Community Participation
     
  • Support Coordination (when part of broader recovery planning)
     

Outcome focus:


Improved wellbeing, stability, social connection and ability to engage in daily life.


How We Deliver Support


All supports are:


  • Practical and real-world based
     
  • Structured and goal-linked
     
  • Consistent and measurable
     
  • Designed to build independence over time
     

We work alongside families, support coordinators, schools and therapists to ensure supports are coordinated and effective.

 

A Note on Core Supports


In some situations, Core supports (e.g. Assistance with Daily Living) may be used as short-term scaffolding to stabilise routines and daily functioning.


As skills develop, supports are reviewed and gradually transitioned back to Capacity Building wherever appropriate.


Our focus is always on building independence over time.
